Il film parla di Amani Ballour, una dottoressa di Ghouta che gestisce un ospedale improvvisato soprannominato "la Grotta" durante la guerra civile siriana.
In un'intervista, il regista ha affermato che il suo scopo nel girare il film era quello di creare prove documentali che potessero essere usate per cercare giustizia per le vittime innocenti della guerra civile. "Il film dovrebbe mettere le persone in una posizione scomoda guardando attraverso la terribile realtà che ci circonda".
La 92esima edizione degli Oscar vedeva, nella categoria miglior documentario, ben due documentari sulla guerra civile siriana. Questo non può essere che un bene: anche questo documentario, dal realizzatore di Last Men in Aleppo, va visto, giusto per ricordare a tutti che la guerra non è mai un bene. Mai.
